Add Schedule Impact and Cost Impact to the RFI cover sheet

When we send a RFI we want the ability to add a cost impact and schedule impact to the cover sheet of the RFI that is being sent out for approval.

    This is pretty important and needs to be implemented ASAP. Almost all RFIs )that don't originate from Team) have some form of this as it is needed to make an informed decision on an RFI.
