While creating a form or at the end of creating a form, have the choice to delete the template if decided against that template.

I agree, we should be able to delete a template and a category. Archiving a template isn't as clean, so I would prefer to be able to delete.
As far as I know there is no ability to delete or archive a category, so if you no longer need a category or create one by accident, it is there forever and becomes an empty category.

You can archive forms now in the Forms Admin page. It was added recently.
